Two questions: Does it build muscle? And how much should I take? Here are the honest answers, and they're simpler than the internet makes them.
Creatine is the rare supplement where the hype has a real core. It does help you build muscle and get stronger. The catch is that it only helps certain people, doing a certain thing, at a dose smaller than most people suggest. Watch the explainer, then keep reading.
The short version
Creatine works, if you lift. With training, creatine adds about a kilogram of muscle and a bit more strength. On its own, without training, a 2022 review found it adds basically nothing (Delpino).
It works for short, hard efforts like lifting and sprints. It does nothing for long-distance running, and the recovery claims are questionable.
Two groups of people run low on creatine: people who eat little or no meat, and older adults who don't get enough protein.
The daily maintenance dose is lower than you think. 3 to 5 grams a day is plenty. The "loading" week of 20 grams per day simply saturates the muscles more quickly.
Without lifting, creatine has no benefit. In a 2-year study, 3 grams a day with no exercise did nothing at all (Sales, 2020).

What it actually does
Think of it as topping off your muscles' fast-energy tank. When you push hard for a few seconds, creatine lets you squeeze out a little more work before you tire. Do that week after week and the extra work adds up to more muscle and more strength.
The gains are modest and real. On short, hard efforts you tend to get 5 to 15 percent more out of yourself. And a 2022 review of the research found that creatine plus lifting adds about 1 kilogram of muscle over lifting alone (Delpino, Nutrition).
What it won't do
It won't make you a better distance runner since that uses a different fuel system, and the studies show no benefit. It might help a little with soreness and recovery, but that evidence is weak and inconsistent. So if anyone sells creatine as an all-around miracle, that's the marketing talking, not the science.
Who actually needs it
There are two groups where supplementing helps, simply because they're starting low.
If you don't eat meat. Almost all the creatine in food comes from meat and fish. Your body makes a little creatine on its own, but not enough to make up for what we'd otherwise get from meat. In one study, women who switched to a vegetarian diet saw their muscle creatine drop within three months. A single gram a day fixed it (Blancquaert, 2018).
If you're older and consuming less protein. As we age we tend to eat less protein and make less creatine, so the tank sits low. Pair that with lifting and this is one of the clearest reasons to take it.
How much you really need
Less than most people suggest. What matters is filling the muscles over time, not how big each scoop is. Three grams a day fills them in about a month. Once they're full, roughly two grams a day keeps them there, as long as you stay consistent.
A "loading" week of 20 grams a day simply fills the muscles quicker, not fuller. Anywhere from 3 to 5 grams a day gets you to the same place; the lower dose just takes a little longer. Five grams a day is simply the most-studied number.
You still have to lift
This is the part the ads skip. Creatine helps your training work better; it doesn't replace it. The clearest proof: a 2-year study gave 200 older women 3 grams a day with no exercise, and it did nothing. No muscle, no strength, no bone benefit (Sales, 2020).
Add the lifting and it works. A 2026 review of seven studies found creatine built muscle and strength in older women but only when they took at least 5 grams a day and trained (Naddafha, 2026). Same powder. The training and enough protein are what turn "topped up" into "stronger."
Bottom line
Creatine builds muscle with training. About a kilogram of muscle and more strength when you lift; without training there is no benefit from creatine.
It's for strength, not endurance. Short, hard efforts benefit; there is no clear benefit for endurance or recovery.
Creatine supplementation matters most if you eat little meat or you're older and low on protein. Vegetarians and older adults are likely starting from a deficit, so they have the most to gain.
3 to 5 grams a day of plain monohydrate is all you need. Skip the fancy forms and the loading.
Take it with lifting and enough protein. Without those two pieces, creatine does nothing.
