Open Instagram or TikTok for thirty seconds and you'll be told creatine is a memory miracle. It's the supplement of the moment and the marketing has raced miles ahead of the evidence.

Creatine is one of the best-studied supplements we have. But almost everyone blurs two very different questions: does it reach your brain, and does it make you think better? Watch the quick explainer:

Does creatine even reach your brain?

Yes, but barely, and slowly. Here's the part most coverage skips: your brain manufactures most of its own creatine. There is a structure called the blood- brain barrier which protects the brain and it is this barrier that limits creatine that is supplemented from entering the brain. That's why supplementing moves brain levels so little while muscle fills up easily.

How little? When Turner and colleagues analyzed the same brain scans two ways in 2015, one method showed a ~6% rise after a week of heavy loading and the other showed no change at all (Magnetic Resonance Imaging). A 2017 analysis (Bhogal, NMR in Biomedicine) showed even the same software gives different answers depending on the settings. Brain creatine probably does rise a little, but the magnitude is small and genuinely uncertain.

Does it actually make you think better?

We don't know yet. Two of the most authoritative reviews of 2024: the European Food Safety Authority (EFSA) and a systematic review by McMorris (Behavioural Brain Research) both concluded the cognitive benefit is not established.

The field is stuck for a simple reason: studies that measured brain creatine almost never measured cognition, and studies that measured cognition almost never measured brain creatine. The dots have never been connected. And the largest, most careful trial by Sandkühler 2023, which included 123 people tried to replicate an earlier positive result and couldn't. Working memory came back borderline and not significant; the other eight tests didn't budge (BMC Medicine).

Then why do some studies look positive?

There are real positive signals but they are small and inconsistent. Rae's 2003 study in 45 vegetarians found gains in working memory (Proc. Royal Society B); McMorris found memory gains in 32 older adults in 2007. But the longest single trial in older women (Alves, 24 weeks) found no cognitive benefit at all (PLoS ONE).

What actually works for your brain?

Aerobic exercise, and it isn't close. In 2025, Singh and colleagues published an umbrella review, which was a meta-analysis of meta-analyses. The study pooled 133 systematic reviews, 2,724 randomized trials and 258,279 people, and found a real, measurable improvement in cognition (British Journal of Sports Medicine).

In addition, the Erickson's 2011 trial, a year of moderate walking grew the hippocampus, the brain's memory hub, by 2%, while the control group's shrank ~1.4%, effectively rolling back one to two years of brain aging (PNAS). Compared with creatine's few trials that cumulatively included a few hundred participants, exercise has more than two hundred times the evidence.

Bottom Line

Creatine is not a proven brain supplement. The cognitive benefit has not been established in humans.

Biology explains why. Your brain makes most of its own creatine and the blood-brain barrier blocks most of the rest. Creatine supplementation barely moves the needle up top.

The best trial was null. Sandkühler 2023 (n=123) found no reliable cognitive gain.

Exercise is the real lever. 150 minutes a week of moderate aerobic activity has orders of magnitude more evidence for your brain than any supplement.
